I have recently encountered a problem with my modded (for PS2)Pad. Everything was working beautifully until it decided to ban me from playing any of the songs (DDR Extreme).
Just recently I had noticed a little tear in the vinyl and I have taped it up since without any problems till 12 hours later, the game went beserk. Starting with "Paranoia Mix" for the 1st stage, the song shut down and "FAILED" appeared on screen followed by GAMEOVER- even before I had the chance to step on ONE ARROW! It didn't even allow me to play the 2nd & 3rd stages of the game. It just restarted the game by itself.
From that moment on NONE of the songs will play.
For navigating through the menus, the pad works at 100% - even for the Party Games it works perfectly. All the lights turn on according to the corresponding arrows. Everything works EXCEPT for letting me play the dance songs.
I've tried using the controller instead (which works) then switching to the pad mid-song and again, it shuts down the game by restarting it. I've tried using the 2nd Player outlet and that too doesn't help.
I have narrowed down the glitch to the cord and/or the box in the pad. Considering I have taken good care of it for the past 2 months I am unsure of what I should do next.
Any ideas? Or is the DDR Mat's fate to R.I.P.?

The other day, I was playing with a pad I got from christmas, in endless mode, i was doing great, then suddenly, it went to the failed screeb. I hope nothing is wrong with my new pad, mayby it just had a little up-chuck. (p.s, my problems were with Extreem also)

FINALLY! It works again!
I didn't think keeping the pad under the sofa bed in the basement would affect the circuits. It appears the humidity had a negative effect on the pad.
Ever since I've kept the pad upstairs, it works like a charm again. ~so happy~
PS: My pad is "modded" as follows:
I've separated the three layers of the vinyl pad except at the main (black)box junction. I removed the bottom black vinyl sheet. The second layer (where the circuits are visible) is adhered in its entirety with 3M 77 Spray Mount to a plastic desk mat (made for carpeting). The top layer (with the arrow graphics) is secured to the same plastic desk mat with rivets all around the edge. This is the best way to keep the bottom contact sheet separated from the top contact sheet (why? see below). It also prevents either contact sheets from creasing, shifting or ripping while jumping gleefully.
The reason I had to do that was because simply attaching the pad with double stick tape wasn't efficient enough. I had issues with the arrows "sticking" (thus registering in the game and making me lose) even though my foot wasn't stepping on it. If it weren't for the red led indicator, I wouldn't have known the connection between the top layer of the matt wasn't being interrupted by stepping off the arrows. The carpet was partly to blame, since it was so thick. The only problem I had after this was when the circuits went bezerk. But keeping it away from cold humid places has resolved this issue.
Thankfully, case closed.
